These are various forms that I have used in my classes over the years.
Take a look, to see if some might be useful to you.
- ATTENDANCE/QUICK QUIZ SHEET  (Microsoft Word document)  
I use this form each week to keep track of the attendance and Quick Quiz scores in my classes.
 
- BLANK INDEX CARD  (Microsoft Word document)  
I pass out ‘in-a-nutshell’ notes to students written on these cards; students then copy them
onto 4"×6" index cards.
 
- BOOK SIGNOUT SHEET    
Have students sign this sheet when you pass out books.
 
- CALCULATOR CONTRACT   
Students and parents must sign this contract in order to be issued a calculator for the year.
 
- DAILY PLANNER SCHEDULE  (pdf file)  
Carol Fisher's daily schedule, sized to cut-and-paste into her daily planner.
 
- FAMILY NIGHT:  
A 10-minute presentation to parents/family about courses; includes a request for parent/guardian contact information.
 
- INDEX CARD GRADING RULES:  
Index cards are passed in and graded at the end of each quarter.
 
- INTRODUCTION TO FISHER WEB SITE:  I use this worksheet at the beginning of school to introduce students to my web site.
 
- MISSED WORK FORM  (Microsoft Word document)  Students
must fill out this form when they miss class work.
 
- NAME SPINNER 
This is a ‘spinner’ to select students in your classes to answer questions.
 Modify this HTML file with your own classes and student names, put a copy on your desktop, and ‘spin’ away!
 
- NUMBER SIGN-UP   
On the first day of class, students choose a number to use for the entire year.
 
- PARENT INFO   
Information needed for parent contact.
 
- PARENT/TEACHER CONFERENCES  
(Microsoft Word file)  Post on your door for students to sign up for conferences.  It has a ‘tear-off
reminder tab’ feature!
 
- RECOMMENDATION REQUEST FORM
  
(Microsoft Word file)  Students fill out this form, if they want me to write them a recommendation.
 
- SIGN-OUT SHEET #1  
(Microsoft Word file)  Students must sign out if they leave class for any reason.
 
- SIGN-OUT SHEET #2  
HTML form; students circle the class that they're in.
 
- SOLVING SENTENCES FORM  
This form makes it easy for students to organize their work when solving mathematical equations and inequalities.
The list of equivalent sentences goes in the first column; supporting reasons go in the second column.
 
- STUDENT GRADE SHEET  (Microsoft
Word document) Students keep a personal grade sheet that is updated every time they get back graded materials, so they
know their grade at every moment in the course.
 
- TEACHER GRADE SHEET  (Microsoft
Word document) Allows for 22 students per class.
 
- TUTORING FORM  Keep track of tutoring sessions.
 
- MATH STUDY TIME CHART  
A student whose grade falls to C- or below may be asked to fill out this chart each week.
 
- 
WEB EXERCISE PRACTICE SHEET  
Students write their answers to web exercises that are projected on the screen in class; if incorrect, they also write the correct answer.
