I agree that I accept full responsibility for the care and condition of this calculator. I agree to pay for the replacement of this calculator at the end of the course if it is not returned in good condition (the condition at the time of loan). The replacement cost of this calculator is shown below. Please note that each calculator has a unique serial number and bar code. Failure to return or pay for the same calculator at the end of the course will result in denial of student's report card and loss of lending and other privileges within the school. The calculator is borrowed from and must be returned to the Library.


The signature of the student and parent/guardian is required before any calculator will be loaned to a student. Signatures indicate agreement with this contract.
